ABOUT BH 365

Behavioral Homes 365 operates three residential homes in Arizona: two in Mesa and one in Queen Creek. We provide structured, evidence-based treatment for adult men and women living with mental health conditions, substance use, and co-occurring disorders, all in a safe, supportive, and nondiscriminatory setting.

Our 90-day program helps clients build the coping skills they need to gain independence, transition back into the community, and lead fulfilling lives with a reduced risk of crisis or relapse.

We believe every client is capable of learning new skills and understanding why they matter, which empowers them to manage their mental health or recovery more effectively. Because each person brings unique abilities, interests, needs, and learning styles, BH365 tailors treatment to the individual and offers a variety of group and individual counseling sessions.

Clients are most ready to learn and adapt when they have a structured daily program built around their needs. That’s why we focus on teaching self-management and helping clients understand how and when to use the resources available to them, so they leave with confidence and a plan.

OUR PROCESS

At BH365, the personal, physical, and spiritual well-being of our clients always comes first. This philosophy guides every part of our care.

We take an interdisciplinary, team-based approach to both how we operate and how we deliver services, so every client benefits from the combined expertise of our entire care team. We also partner with local and state nonprofits and government agencies to connect clients with the resources they need and to better serve our community.

ADMISSION AND SCREENING

Most clients come to BH365 through a referral source, such as inpatient (Level I) facilities, hospitals, behavioral health programs, physicians, case managers, social workers, health plans, or family members. Working with the referral source helps our team understand each client’s behavioral health needs, substance use history, and any co-occurring conditions before they arrive.

At admission, a Registered Nurse completes a nursing assessment, and a Behavioral Health Professional completes a behavioral health assessment. Together, these assessments give us what we need to create an individualized treatment plan from day one.
