Admission criteria

  • Adults both male and female 18 years and older with mental illness, substance abuse diagnosis, alcohol addiction, dual diagnosis or co-occurring disorders.
  • Indication that the individual can benefit from the program and its services.
  • The ability to reside in an open setting.
  • Compatible behavior that does not pose significant risk or danger to the well-being of self or others.
  • Completion of financial or insurance plan arrangements.
  • An admission assessment completed by a Registered Nurse.
  • A behavioral health admission assessment completed by a Behavioral Health Professional.
  • Limits the residents ability to be independent or cause the residents to require treatment to maintain or enhance independence.

Discharge criteria

  • The deterioration in an individual’s physical or psychological condition requiring specific services not available through BH 365. Requiring higher level of care.
  • Disruptive and inappropriate behavior by the individual that poses significant risk or danger to the well-being of self or others.
  • No longer benefits from or needs the program and its services. Has gained independence and has properly completed treatment.
  • Loss of authorization from the payer or inability to pay for services. BH365 will work with other facilities to refer the resident. BH365 will cover the cost during transition so the resident is not left without a place to go.
  • Achieves the goals as measured by the resident, determined by the behavioral health professional and Registered Nurse.
  • Attending to ADL’s, compliant with medication regimen, understands and demonstrates when behavioral services are needed, maintains physical and psychological functioning.
  • Self-determination of any resident who is not under court order or guardianship to terminate from the program.
